Guidelines for filling up Change of Name Notification Form |
|
- Fields marked with *are mandatory.
- Use only A to Zcharacters while entering Name.
- Use only A to Z, a to z, 0 to 9 alphanumerics, comma (,) while entering information in the Address field. Do not use any other special characters. 100 characters only.
- Please make sure that you have Valid Email ID, if you do not have an email id create a new one.
- Do not use prefix 0 (Zero)to Mobile Number.
- Ensure file size between 100 KB to 200 KB file type of jpg for Change of Name Form upload Marksheet, Principal attested Letter, Passport size Photo and Signature.
- Upload clear images.
- Do not enter dash (-)for the fields which are not applicable or blank.
- For Change of Name Notification 200/- (All the fees payments are non-refundable)
