| Guidelines for filling up Date of Declaration Form |

- Fields marked with *are mandatory.
- Use only A to Zcharacters while entering Name.
- Use only A to Z, a to z, 0 to 9 alphanumerics, comma (,)while entering information in the Address field. Do not use any other special characters. 100 characters only.
- Please make sure that you have Valid Email ID, if you do not have an email id create a new one.
- Do not use prefix 0 (Zero)to Mobile Number.
- Ensure file size between 100 KB to 200 KB
and file type of .jpg for Date of Declaration Form upload Passport size Photo , Signature and Semester Marksheets.
Upload clear images.
Do not enter dash (-)for the fields which are not applicable or blank.
For Date of Declaration verification 200/- (All the fees payments are non-refundable)
